At My Team Identity, we make ordering customized nameplates simple so you can focus on what truly matters: saving lives.
There are 9 DESIGN TEMPLATES to choose from. Simply select your favorite DESIGN TEMPLATE and our designers will substitute your logo and add firefighter information as requested. OR, select 'DESIGNERS CHOICE' and our designers will select the best option for you.
Within 1-2 business days of placing your order we will email you a design proof from our automated approval system. Click on the link in the email to view the design proof and then select either 'approve' or 'request edits' and type in the changes. If approved, your order automatically gets added to the printer line up and will usually print within 1-2 business day and ship. If you requested edits you will receive another design proof within 1 business day with the requested changes. We will NOT print until you have approved the design. Please note that requesting edits will delay your order fulfillment by one day per requested edit.

MOUNTING OPTIONS:
Our plastic nameplates do not include any mounting option. Most popular are the double sided adhesive pads or velcro strips. Both can be purchased at an hardware store, crafting store or online through Amazon.
ORDER MINIMUMS:
-A MINIMUM ORDER OF 14 NAMEPLATES is required for your initial order. If the order is less than 10 nameplates your order will be cancelled and fully refunded.
BULK ORDERS:
For any order of more than 25 nameplates please email us for a price quotation at info@myteamidentity.com. We offer excellent bulk discounts to our customers with larger orders.
RUSH ORDERS:
Please contact us prior to purchasing a rush order. The DATE NEEDED must be indicated in the Personalization notes. If the date is not indicated there it is not guaranteed they will arrive on time. You will also need to purchase an expedited shipping method.
REPEAT ORDERS:
For future orders using the same design please order from our website at www.myteamidentity.com and click on the red "RE-ORDER" link in the top right hand corner. Answer all the questions and you can easily place the order for more bag tags with no order minimums. Please note that the background and design will be identical to the order indicated at purchase. You will automatically receive a 10% discount.
RETURNS OR EXCHANGES:
We do not accept refunds on custom items. You will receive a design proof for approval. The final item will look identical to the design proof. If you item arrives damaged contact us right away and we will file a claim with USPS and send out replacements.
SHIPPING:
Orders will be shipped for free via USPS Priority Mail which takes 3-5 business days. A tracking number will be sent to you once the label is processed.
